Welcome to Civil Surgeon
Civil Surgeon is a website that helps you complete USCIS Form I-693 (the Report of Immigration Medical Examination and Vaccination Record). It does the math and the paperwork for you, so you can finish a form in minutes instead of hours.
How it works, in plain words
The work happens in two parts:
- The patient fills out their own information. You send each patient a link. They type in their name, date of birth, address, and phone number on their phone or computer — in English or Spanish.
- You finish the medical parts. The patient's information shows up on your Dashboard. You open it, add the exam findings and vaccine records, and download a finished PDF that is ready to print and sign.
tip
You only have to set up your account once. After that, every new patient is just three steps: they fill out their part, you fill out the medical part, and you download the PDF.
What you will need
- An email address and a password.
- A payment card (there is a 7-day free trial first — see Start Your Free Trial).
- Your Civil Surgeon ID (CSID) number from USCIS.
